Care Led by Experienced Physical Therapists

The ECPT Standard

Elliott Concierge Physical Therapy was built for clients who want a better way to recover — more personal than a traditional clinic, more convenient than traveling to appointments, and more focused than rushed, insurance-driven care.

Every session is one-on-one and designed around your goals, your environment, and your schedule. In South Bend, that may mean care in your home, office, private gym, senior living residence, or another setting that is convenient and appropriate for your care.

Founded by Matthew Elliott, PT, DPT

Matthew Elliott is a Doctor of Physical Therapy and the founder of Elliott Concierge Physical Therapy. After seeing how traditional physical therapy can feel rushed, crowded, and disconnected from the way people actually live, he created ECPT around private care, clear communication, and thoughtful clinical progression.

Our goal is to help clients feel better, move better, and return to the activities that matter most — without unnecessary friction, wasted time, or a generic clinic experience.

Why Concierge Physical Therapy Works So Well in South Bend

In South Bend, getting high-quality physical therapy should not mean sitting in a waiting room, driving across town after surgery, or settling for a rushed clinic experience.

Our model removes that friction. We bring expert care to you, work in the environment where you actually live and move, and build a plan around your goals, your schedule, and your lifestyle.

For some clients, that means post-surgical rehab at home. For others, it means balance training, mobility work, hands-on treatment, or a long-term strength plan designed to keep them active and independent.

South Bend is also a relationship-driven community with strong anchors in higher education, healthcare, manufacturing, and local business. Major regional employers include the University of Notre Dame, Beacon Health System, Saint Joseph Health System, AM General, Honeywell, Indiana University South Bend, and others, which supports a broad base of professionals, retirees, medical families, and executives who may value private, convenient healthcare options.

How to get started

  • Call or text now to schedule . Not ready to schedule? Navigate to the “Request Care” tab, and sign up for a free phone consultation to further discuss your case, and what we can do to help.

  • During your initial evaluation, your therapist will assess your symptoms, movement, strength, mobility, balance, and goals. The purpose is to identify what is limiting you and create a plan tailored to your needs, your environment, and your daily life.

This may include balance testing, flexibility assessment, strength testing, orthopedic special tests, gait analysis, and a review of your surgical history, imaging, or physician recommendations when applicable.

  • Treatment sessions are tailored to your goals and may include hands-on therapy, mobility work, strengthening, balance training, gait training, corrective exercise, education, and progression back to the activities that matter most to you.

Many clients notice a meaningful improvement after the first session because the care is focused, individualized, and delivered one-on-one.

  • After just a few sessions, many clients are moving better, feeling stronger, and returning to the activities they enjoy. Whether your goal is walking comfortably, recovering after surgery, playing golf, traveling, caring for family, or staying independent, the time to start is now.

Frequently Asked Questions

Can I use my insurance?

Yes, we utilize your out-of-network benefits in order to decrease your cost of care whenever possible. For patients without out-of-network benefits who still want to receive one-on-one skilled physical therapy, we provide single visit rates and package options.

Do you accept Medicare?

Elliott Concierge Physical Therapy is a private-pay and out-of-network concierge physical therapy practice. If you are a Medicare beneficiary, please contact us before scheduling so we can determine whether our services are appropriate for your situation and explain your options clearly.

This is important because Medicare has specific rules around physical therapy coverage, private-pay services, and home health eligibility.

How long will each session last?

The initial session lasts a full 60 minutes. After that, you may have the opportunity to choose between 30, 60, and 90-minute sessions depending on your case, your goals, and your availability.

Where do sessions take place?

Your therapist comes to you. Sessions may take place in your home, office, private gym, senior living residence, or another appropriate private space.

Your therapist will bring the necessary equipment. In most cases, all you need is roughly a 6’ x 6’ open area.

What does a session look like?

Sessions vary from person to person. For most clients, treatment includes a combination of hands-on therapy, movement assessment, corrective exercise, strengthening, balance or mobility work, and education.

Depending on your case, care may also include reviewing imaging, coordinating with your physician, helping you understand your surgical precautions, or building a long-term plan to stay active and independent.

Do I need a physician referral?

In many cases, you can begin physical therapy without a physician referral. However, depending on your medical history, surgery, diagnosis, insurance situation, or state-specific requirements, we may recommend or require coordination with your physician.

If you recently had surgery, we are happy to review your physician’s protocol or post-operative instructions.

What areas do you serve?

We serve South Bend, Granger, Mishawaka, the Notre Dame area, and select surrounding Northern Indiana / Michiana communities by application. Availability depends on location, therapist schedule, and travel distance.

Is concierge physical therapy right for me?

Concierge physical therapy is a good fit if you want private, one-on-one care, prefer not to travel to a clinic, value convenience, and want a more personalized experience than traditional outpatient physical therapy.

It may be especially helpful after surgery, during a pain flare-up, when balance or mobility has declined, or when you want expert guidance to stay active and independent.
