In-Home Physical Therapy in South Bend, Granger, and Mishawaka

Expert one-on-one physical therapy brought to your home, office, private gym, or residence - designed for South Bend area clients who value convenience, privacy, and a higher standard of care.

Physical Therapy Built Around Your Life

If you are recovering from surgery, managing pain, rebuilding strength, improving balance, or trying to stay active, getting the right care should not require rearranging your entire day.

ECPT brings expert, one-on-one care directly to your home, office, private gym, or residence in the South Bend area. Every visit is provided by a licensed Doctor of Physical Therapy—without handoffs to technicians or assistants.

Private-pay physical therapy is not about paying more for the same clinic experience. It is about a more focused model: direct communication between visits when appropriate, less travel and disruption, and a plan built around your goals and response to care. For some patients, that may mean fewer visits per week and less time away from work or family.

The right schedule depends on your condition and goals. We focus on providing the right care—not simply the most visits.

Why the ECPT Model May Be Worth It

High-quality physical therapy should not require a crowded clinic, a long commute, or more visits than your care actually needs.

ECPT brings a Doctor of Physical Therapy directly to you in South Bend, Granger, Mishawaka, the Notre Dame area, and select surrounding communities. Your care is one-on-one, without technicians or assistants, and your plan is built around your goals, your environment, and your progress.

Private-pay care may be worth it for people who value focused clinical attention and a more efficient experience. Direct communication between visits, when appropriate, can help keep your plan moving forward. Less travel can mean less time away from work or family, and some patients may be able to schedule fewer visits per week or reduce the total time and expense involved in getting care.

The right schedule depends on your condition and goals. Our focus is the right care—not simply the most visits.

Do you want care shaped around your goals—or limited by what insurance will cover?

Care Led by Experienced Physical Therapists

The ECPT Standard

Elliott Concierge Physical Therapy was built for clients who want a better way to recover — more personal than a traditional clinic, more convenient than traveling to appointments, and more focused than rushed, insurance-driven care.

Every session is one-on-one and designed around your goals, your environment, and your schedule. In South Bend, that may mean care in your home, office, private gym, senior living residence, or another setting that is convenient and appropriate for your care.

Founded by Matthew Elliott, PT, DPT

Matthew Elliott is a Doctor of Physical Therapy and the founder of Elliott Concierge Physical Therapy. After seeing how traditional physical therapy can feel rushed, crowded, and disconnected from the way people actually live, he created ECPT around private care, clear communication, and thoughtful clinical progression.

Our goal is to help clients feel better, move better, and return to the activities that matter most — without unnecessary friction, wasted time, or a generic clinic experience.

Frequently Asked Questions

Can I use my insurance?

ECPT is a private-pay physical therapy practice. Payment is due at the time of service. Superbills are provided to patients with commercial insurance who wish to pursue reimbursement through out-of-network benefits.

Do you accept Medicare?

No. ECPT does not participate in Medicare.

Payment and reimbursement details are discussed before care begins.

How long will each session last?

The initial session lasts a full 60 minutes. After that, you may have the opportunity to choose between 30, 60, and 90-minute sessions depending on your case, your goals, and your availability.

Where do sessions take place?

Your therapist comes to you. Sessions may take place in your home, office, private gym, senior living residence, or another appropriate private space.

Your therapist will bring the necessary equipment. In most cases, all you need is roughly a 6’ x 6’ open area.

What does a session look like?

Sessions vary from person to person. For most clients, treatment includes a combination of hands-on therapy, movement assessment, corrective exercise, strengthening, balance or mobility work, and education.

Depending on your case, care may also include reviewing imaging, coordinating with your physician, helping you understand your surgical precautions, or building a long-term plan to stay active and independent.

Do I need a physician referral?

In many cases, you can begin physical therapy without a physician referral. However, depending on your medical history, surgery, diagnosis, or state-specific requirements, we may recommend or require coordination with your physician.

If you recently had surgery, we are happy to review your physician’s protocol or post-operative instructions.

What areas do you serve?

We serve South Bend, Granger, Mishawaka, the Notre Dame area, and select surrounding Northern Indiana / Michiana communities by application. Availability depends on location, therapist schedule, and travel distance.

Is concierge physical therapy right for me?

Concierge physical therapy is a good fit if you want private, one-on-one care, prefer not to travel to a clinic, value convenience, and want a more personalized experience than traditional outpatient physical therapy.

It may be especially helpful after surgery, during a pain flare-up, when balance or mobility has declined, or when you want expert guidance to stay active and independent.
