Vestibular & balance care

Not All Dizziness Is the Same

Dizziness and vertigo can come from different causes, and the right treatment depends on what your examination shows. BPPV is one common cause of positional vertigo, but it is not the only reason someone may feel dizzy or unsteady.

Find Out What Your Symptoms Point To

Your visit includes a focused look at your symptoms, balance, eye and head movements, walking, and positional triggers to see whether vestibular physical therapy is appropriate and what may help.

Treatment Can Begin the Same Visit

When appropriate, care starts during your first appointment: BPPV repositioning maneuvers, vestibular exercises, balance training, and walking practice.

No Driving While Dizzy

Vertigo and imbalance can make driving—or even riding in a car—uncomfortable or unsafe. Your physical therapist comes to you, so you're evaluated where you actually move.

What we evaluate

Dizziness, Vertigo & Balance Symptoms We Evaluate

You don't need to know what is causing your symptoms before contacting us. Your evaluation is designed to find out whether physical therapy is appropriate and what type of treatment may help.

Positional Vertigo / Possible BPPV

  • Spinning when rolling over in bed
  • Vertigo when getting into or out of bed
  • Dizziness when looking up or bending forward
  • Brief spinning episodes triggered by certain head positions

Dizziness With Head or Body Movement

  • Dizziness when turning your head
  • Feeling off when walking and looking around
  • Motion sensitivity
  • Symptoms that increase in busy or visually stimulating places

Balance & Unsteadiness

  • Feeling unsteady while standing or walking
  • Difficulty turning or changing direction
  • Reduced confidence on stairs or uneven ground
  • Falls or near-falls, or feeling less stable in the dark

Lingering Vestibular Symptoms

  • Persistent symptoms after a vestibular episode
  • Difficulty focusing while your head is moving
  • Ongoing imbalance after an inner-ear problem
  • Residual dizziness after treatment for BPPV

Not every cause of dizziness is appropriate for physical therapy. If your examination suggests your symptoms need additional medical evaluation, we'll help guide you to the appropriate next step.

Your first visit

What to Expect From Your Vestibular Evaluation

Your first visit is a 60-minute, one-on-one evaluation and treatment session in your home with a Doctor of Physical Therapy. Before it ends, you'll understand what we found, what we recommend, and what your next step should be.

  • We listen to your symptoms. When the dizziness started, what it feels like, how long it lasts, and which movements—rolling in bed, looking up, bending over, turning your head—bring it on.
  • We perform a focused vestibular and balance exam. Depending on your symptoms: positional testing for BPPV, eye and head movement testing, gaze stability, standing balance, walking, strength, and fall-risk assessment.
  • When appropriate, treatment starts right away. You usually don't have to wait for another appointment to begin care.
  • If physical therapy isn't the right starting point, we'll recommend the appropriate medical follow-up rather than treating something outside our scope.

Treatment

Treatment Based on What We Find

Vestibular care shouldn't be a generic set of balance exercises. Your plan is chosen from your symptoms and examination, and it changes as you respond.

BPPV Repositioning

Canalith repositioning maneuvers when testing points to BPPV.

Gaze-Stabilization Training

Exercises that help your eyes stay steady while your head moves.

Habituation

Graded exposure to the movements and settings that provoke your symptoms.

Balance & Walking Training

Practice in the hallways, stairs, and rooms you use every day.

A Personalized Home Program

Clear exercises and strategies to keep progress going between visits.

Coordination With Your Medical Team

When appropriate, we coordinate with your physician, ENT, neurologist, or other provider.

Some vestibular problems respond quickly; others need a more progressive program. Your recommendations are based on your findings and how you respond—not on a set number of visits.

What Patients Say About Vestibular Care

“Matt came to my home and spent a lot of time figuring out what was contributing to my symptoms. He explained everything clearly and gave me exercises that actually felt specific to what I was experiencing instead of just general balance exercises. Within a few visits, I could tell things were improving. I was less dizzy, felt steadier on my feet, and started feeling more comfortable doing things on my own again.”

Questions

Questions About Vestibular Physical Therapy

Do I need to know whether I have BPPV?

No. You don't need to know the cause of your dizziness or have a diagnosis before contacting us. We'll talk through your symptoms and whether an in-home vestibular evaluation appears appropriate.

Can treatment begin during the first visit?

When appropriate, yes. The first visit includes both evaluation and treatment. If your findings suggest that additional medical evaluation is needed first, we'll explain the next step and help guide you to the appropriate provider.

How long are visits?

Your first visit is a full 60 minutes, one-on-one with a Doctor of Physical Therapy. After that, visits run 45, 60, or 90 minutes, depending on your plan, goals, and availability.

Do I need a referral or prescription?

In many cases, you can begin physical therapy without a physician referral. However, depending on your medical history, surgery, diagnosis, insurance situation, or state-specific requirements, we may recommend or require coordination with your physician.

Do you bill insurance or Medicare?

ECPT is a private-pay practice and does not bill Medicare. For patients with commercial insurance, we provide an itemized invoice (often called a superbill) to submit for out-of-network reimbursement. Reimbursement depends on your plan.

Where do visits take place?

Your therapist comes to you—your home, office, private gym, senior living residence, or another appropriate private space—in South Bend & Michiana, Southwest Michigan, and Naples & Southwest Florida.
