Serving Southwest Michigan
In-Home Physical Therapy in Niles, Edwardsburg, and Cassopolis
A Doctor of Physical Therapy comes to your home, lake house, office, or private gym, so you can recover in southwest Michigan without driving to a clinic.
Expert Care Without the Drive
In Niles, Edwardsburg, and Cassopolis, good physical therapy often means a drive, a waiting room, and a schedule built around the clinic. That is a lot to ask when you are recovering from surgery, dizzy, in pain, or simply busy.
ECPT brings one-on-one care to you instead. Every visit is with a Doctor of Physical Therapy licensed in Michigan, never handed off to a technician or assistant.
Private pay is not about paying more for the same clinic visit. It is a more focused model: unhurried one-on-one visits, direct communication between visits when appropriate, and a plan built around your goals and how you respond. For some patients, that means fewer visits and less time away from work or family.
How often you are seen depends on your condition and goals, not on filling a clinic schedule.

Why the ECPT Model May Be Worth It
High-quality physical therapy should not require a crowded clinic, a long drive, or more visits than your care needs.
ECPT brings a Doctor of Physical Therapy directly to you in Niles, Edwardsburg, and Cassopolis, as part of our care across Michiana. Your care is one-on-one, without technicians or assistants, and your plan is built around your goals, your environment, and your progress.
Private-pay care may be worth it if you value focused clinical attention and a more efficient experience. Skipping the drive saves time on every visit, and staying in touch between visits, when appropriate, keeps your plan moving. Some patients need fewer visits per week, which can lower the total time and cost of care.
We plan for the care you need, not the most visits.
Do you want care shaped around your goals—or limited by what insurance will cover?
Care Led by Experienced Physical Therapists
The ECPT Standard
Elliott Concierge Physical Therapy was built for clients who want a better way to recover — more personal than a traditional clinic, more convenient than traveling to appointments, and more focused than rushed, insurance-driven care.
Every session is one-on-one and designed around your goals, your environment, and your schedule. In southwest Michigan, that may mean care in your home, a lake house, your office, a private gym, a senior living residence, or another setting that is convenient and appropriate.
Founded by Matthew Elliott, PT, DPT
Matthew Elliott is a Doctor of Physical Therapy, licensed in Michigan, and the founder of Elliott Concierge Physical Therapy. After seeing how traditional physical therapy can feel rushed, crowded, and disconnected from the way people actually live, he created ECPT around private care, clear communication, and thoughtful clinical progression.
Our goal is to help clients feel better, move better, and return to the activities that matter most — without unnecessary friction, wasted time, or a generic clinic experience.

Frequently Asked Questions
Can I use my insurance?
ECPT is a private-pay physical therapy practice. Payment is due at the time of service. We provide an itemized invoice (often called a superbill) for patients with commercial insurance who wish to pursue reimbursement through out-of-network benefits.
Do you accept Medicare?
No. ECPT does not participate in Medicare.
Payment and reimbursement details are discussed before care begins.
How long will each session last?
The initial session lasts a full 60 minutes. After that, you may have the opportunity to choose between 30, 60, and 90-minute sessions depending on your case, your goals, and your availability.
Where do sessions take place?
Your therapist comes to you. Sessions may take place in your home, lake house, office, private gym, senior living residence, or another appropriate private space.
Your therapist will bring the necessary equipment. In most cases, all you need is roughly a 6’ x 6’ open area.
What does a session look like?
Sessions vary from person to person. For most clients, treatment includes a combination of hands-on therapy, movement assessment, corrective exercise, strengthening, balance or mobility work, and education.
Depending on your case, care may also include reviewing imaging, coordinating with your physician, helping you understand your surgical precautions, or building a long-term plan to stay active and independent.
Do I need a prescription in Michigan?
In Michigan, you can see a physical therapist without a prescription for up to 21 days or 10 treatments, whichever comes first. To continue after that, Michigan law requires a prescription from your physician or another qualified prescriber, such as a nurse practitioner. Care that is only for injury prevention or fitness does not have this limit. We will tell you up front whether this applies to you.
If you recently had surgery, we are happy to review your physician’s protocol or post-operative instructions.
What areas do you serve?
In southwest Michigan, we serve Niles, Edwardsburg, and Cassopolis. We also serve South Bend, Granger, Mishawaka, the Notre Dame area, Osceola, and Elkhart in Indiana. Availability depends on location, therapist schedule, and travel distance. If your town is not listed, call or text and ask.
Are you licensed in Michigan?
Yes. Matthew Elliott, PT, DPT, is licensed to practice physical therapy in Michigan.
Is concierge physical therapy right for me?
Concierge physical therapy is a good fit if you want private, one-on-one care, prefer not to travel to a clinic, value convenience, and want a more personalized experience than traditional outpatient physical therapy.
It may be especially helpful after surgery, during a pain flare-up, when balance or mobility has declined, or when you want expert guidance to stay active and independent.
