Physical Therapy Built Around the Patient—Not the Clinic
One-on-One Attention
Your visit is centered on you, your goals, and how you are responding to treatment—not on managing several patients at the same time.
Care Where Life Happens
Receive physical therapy in your home, workplace, fitness setting, or another appropriate location without adding unnecessary travel, waiting rooms, or clinic logistics.
Clinical Depth
ECPT treats a broad range of orthopedic, post-surgical, vestibular, neurologic, mobility, TMD, and performance-related needs.
Care Shaped Around You
Your treatment is built around your goals, your environment, and how you respond—not around a generic clinic routine.
Good physical therapy requires more than exercises and equipment. It requires time, clinical judgment, communication, and an understanding of how a person actually moves through everyday life.
ECPT brings that care directly into the environment where you live, work, recover, and return to the activities that matter.
Physical therapy needs rarely fit neatly into one category. ECPT evaluates the person in front of us and develops care around the problems, goals, environment, and activities that matter most.

For People Who Want More From Physical Therapy
You're recovering after surgery
and would rather begin or continue rehabilitation where you live.
Getting to a clinic is difficult
because of mobility, balance, neurologic symptoms, transportation, or caregiver logistics.
Your schedule is demanding
and you want high-quality care without repeatedly losing time traveling to appointments.
You want more individualized attention
than you have experienced in a traditional high-volume clinic.
You have a more complex problem
and value a clinician who can spend time understanding the whole picture.
Staying active matters to you
whether that means golf, pickleball, fitness, travel, work, caring for family, or simply moving confidently through daily life.
The common denominator is not age or diagnosis. It is wanting thoughtful physical therapy delivered in a more personal way.

“I had been dealing with dizziness and balance issues for months. I was nervous walking outside, turning my head too quickly, and even getting up at night because I was worried I might fall.
Matt came to my home and spent a lot of time figuring out what was causing my symptoms. He explained everything clearly and gave me exercises that actually seemed specific to what I was experiencing.
Within a few visits I could tell things were improving. I was less dizzy, felt steadier on my feet, and started feeling more comfortable doing things on my own again. Having Matt come to my home also made everything much easier.”

A Company Designed Around Better Care
Elliott Concierge Physical Therapy was built around a simple idea: physical therapy can be highly personal, clinically thoughtful, and significantly more convenient than the traditional clinic experience.
Rather than asking every patient to fit into the same setting and schedule, ECPT brings individualized physical therapy into the environment that makes sense for the person receiving it.
Today, ECPT serves patients across South Bend and Michiana and Naples and Southwest Florida, with care delivered by experienced physical therapists committed to individual attention, clinical quality, and strong communication.
ECPT's long-term goal is not simply to provide convenient physical therapy. It is to build a trusted healthcare organization capable of delivering an exceptional and consistent patient experience across the communities we serve.
Matthew Elliott, PT, DPT
Founder, Elliott Concierge Physical Therapy
Matt founded ECPT to create a model that gives physical therapists the ability to provide highly individualized care while making excellent physical therapy easier for patients to access.
A Responsive Option for Your Patients
For physicians, surgeons, dentists, concierge medical practices, and other healthcare professionals.
When a patient would benefit from individualized physical therapy but mobility, transportation, schedule, privacy, complexity, or convenience makes conventional outpatient care less practical, ECPT provides another option.
We welcome conversations about patient fit and interdisciplinary care.
Post-Surgical Care at Home
For appropriate patients who would benefit from rehabilitation in their home environment.
Vestibular & Neurologic Needs
For patients whose dizziness, balance, mobility, or neurologic symptoms make travel more difficult.
TMD & Orofacial Collaboration
Physical therapy that can complement appropriate dental, airway, oral-surgery, and multidisciplinary treatment.
Direct Clinical Communication
With appropriate patient permission, ECPT can coordinate care with referring and treating providers.
